Why Does My Honda Foreman 450s Jump Out Of 1st And 2nd Gear Under Acceleration?

I have a 2004 Honda Foreman 450 S with 1516 miles and 260 ish hours. Under even moderate acceleration it will jump out of gear, or not shift into it entirely. Does anyone have any tips or advice on how to remedy this situation?
